Description The Licensed Behavioral Health Specialist ( BHS ) is a key behavioral health provider embedded as a core member of the multi‑disciplinary integrated care team, led by the primary care provider, and including clinical nursing staff, case management, pharmacy and psychiatrists. The role of the BHS is to help the primary care provider and team improve their effectiveness and efficiency in managing the whole patient, focusing on the mental, behavioral and psychosocial challenges that often emerge in the primary care setting. Responsibilities Assist the primary care provider and interdisciplinary team in managing patients, focusing on mental, behavioral, and psychosocial challenges. Provide behavioral health assessment, counseling, and care coordination to improve patient outcomes. Document clinical assessments, interventions, and progress in the medical record in a timely and accurate manner. Collaborate with clinical staff, including nurses, pharmacists, and psychiatrists, to develop and implement comprehensive care plans. Ensure compliance with clinical guidelines, state licensing requirements, and quality metrics. Required Qualifications Master’s degree in Counseling, Psychology, or Social Work. California Clinical Social Worker License upon hire. 3 years of related work experience. Experience working with underserved, vulnerable populations. Preferred Qualifications 4 years of related work experience in a primary care clinic or hospital setting. Leadership experience.
