Registered Nurse (RN) - Home Health

In Home Health, you provide skilled nursing care to patients in their residences, focusing on assessment, care planning, and patient education to improve health outcomes. You work as part of an interdisciplinary team to deliver ongoing management of chronic and acute conditions in the home setting.

Responsibilities

• Conduct home visits to assess health status and update care plans
• Administer medications, wound care, injections, and IV therapies as applicable
• Monitor vitals, manage chronic diseases, and coordinate with providers
• Educate patients and caregivers on self-management, safety, and disease control
• Document assessments, interventions, and outcomes in the EHR
• Coordinate with case management, physical therapy, and social work for comprehensive care
• Respond to changes in patient condition and escalate as needed

Qualifications

Education:

BSN preferred; ADN or nursing diploma acceptable with relevant home health experience

Licensure:

Current, active Registered Nurse license in the United States

Certifications:

• Basic Life Support (BLS)
• Home health-specific certifications as available
• CPR certification

Experience or Skills:

Experience delivering nursing care in home health or community settings; strong independence, time management, and communication skills

Experience

• Experience providing skilled nursing in a home health program
• Experience with IV therapy, wound care, and patient education
• Experience using home health documentation and telehealth or remote monitoring tools

Preferred

• Home health certifications or case management experience
• Spanish-speaking or culturally diverse patient care experience

Work Environment

Travel to patient homes; autonomous schedule with accompanying on-call components

Documentation and communication with a central office and supervising clinician are essential; priorities include safety and patient quality of life
