LPN- Opioid Clinic - Per DiemHorizon Health is seeking a LPN for our clinic in Petersburg, VA. The OTP LPN delegates, coordinates, and evaluates patient care through collaborative efforts with the interdisciplinary team members and in accordance with the nursing process and the OTP standards of care and practices.Responsibilities:Dispensing Methadone and other medication, per physician orders and per clinic policyAccurately record all patient information in the patient recordCompletes brief assessment of patient's mental and physical well-being, monitoring for signs/symptoms associates with licit and illicit drug useParticipates in UDS surveillance, per policyCoordinates medication reviews and staffing with agency physiciansCompletes all required controlled substance inventories to ensure accurate medication countsConducts patient education programs according to needs identified, including health and wellness educationParticipates in treatment team meetings, providing feedback to the interdisciplinary teamSchedule Saturday's 5a-9aHorizon's programs have dedicated marketing support and outstanding resources from the nation's largest psychiatric contract management company.
