HUD/SOAR SupervisorSea Mar Community Health Centers, a Federally Qualified Health Center (FQHC) founded in 1978, is a community-based organization committed to providing quality, comprehensive health, human, housing, educational and cultural services to diverse communities, specializing in service to Latinos in Washington State. Sea Mar's network of services includes more than 90 medical, dental, and behavioral health clinics and a wide variety of nutritional, social, and educational services. We are recruiting for the following position:The HUD/SOAR Supervisor oversees the HUD Housing and SOAR programs. This individual supervises the HUD Housing Case Managers and SOAR personnel in all phases of their work, provides direct services to clients, conducts all SOAR trainings and supervises permanent supportive housing contracts. The person in this position also supervises the HUD and SOAR support staff in the implementation of systems, and coordinates with the Behavioral Health Program Manager (Community Support Services) regarding administrative and clinical management procedures relating to clients. Reviewing client charts to ensure compliance with BHO, DBHR, and TJC (The Joint Commission) requirements.Provide individual and group supervision to all HUD and SOAR case managers and program staff working on your site.Review and approve the case manager's documentation, as appropriate.In partnership with the manager, ensure staff meets their weekly service hours and monthly productivity requirements.In collaboration with the manager, ensure staff has full caseloads and that caseloads are adequately distributed among all staff.Ensure client retention and treatment success.Ensure all staff follows the Sea Mar patient-centered model.Conduct weekly file reviews and implement appropriate corrective actions to ensure files meet Federal, State, County, and Sea Mar requirements. Report all findings to the Program Manager and, as appropriate, to the Sea Mar - CSNW Behavioral Health Director.In collaboration with the manager, ensure QI reports are completed on a timely manner.Maintain a positive relationship with the BHO and referral agencies.Promote the program to community based agencies, faith communities, social services agencies, criminal justice institutions, community health centers, schools, and any other potential referral agency.In collaboration with the manager, ensure clinical staff submits daily collected data and daily data entry in the regional data system. Ensure data timeliness and accuracy.In partnership with the Program Manager, ensure client information is protected and that all employees follow the HIPPA confidentiality requirements.Work in partnership with the Program Manager to ensure high quality of care for all Sea Mar patients.Ensure daily completion of electronic progress notes. Review and approve electronic progress notes, when necessary, in a timely fashion.Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.Demonstrate knowledge of the principles of growth and development over the life span.Able to identify and categorize each patient's age-specific grouping of needs, such as those for infant, adolescent, or geriatric patients.Provide supervision of case managers as required to assure that care is provided according to the WAC, county contracts, TJC, and Sea Mar Policies and Procedures.Ensure system integration of care (primary care, behavioral health, dental care and overall health) for all patients.Participate in management meetings and staff meetings in order to coordinate effectively with other members of staff and management team, assists in making appropriate inter-agency referrals.Provide assessments and individual services to individuals.Maintain regular contact with clients, including home visits, if necessary in order to assess and evaluate the consumer's basic needs, strengths and weaknesses.Consult and collaborate with other professionals/agencies on client evaluations, services plans and follow-up.In collaboration with manager, prepare concise, complete and timely consumer records, according to agency policy and WAC requirements.Assure that their clinical staff maintains updated documentation as required by WAC, County and State contracts.Assure that all clinical staff meets TJC standards in the delivery of clinical services.All other duties as assigned by the Sea Mar - CSNW Director.Conditions of EmploymentThis person must sign an oath of permanent confidentiality covering all patient related information.This person must receive a background check from DSHS and Washington State Patrol.This person must comply with all ethical standards of Sea Mar and as required by DOH.The person in this position shall have no history or evidence of alcohol or other drug misuse for a period of two (2)years immediately prior to the date of employment at the facility, and no misuse of alcohol or other drugs while employed at this center.This person must complete an annual competency test.The Sea Mar - CSNW Regional Director shall sign and date the job description to affirm the validity of the information herein.QualificationsTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Will consider substitution of work experience for degree on individual basis.Agency Affiliated Counselor in the State of WashingtonMental Health Professional; certification as minority Mental Health Specialist as prescribed by WAC, is preferredAt least five (5) years' experience in the field, with some administrative experience.ADDITIONAL REQUIREMENTSMust have and maintain a current CPR certification. If not active at time of hire, must obtain within 90 days of hire. Must have and maintain active First Aid certification.Must take a TB test annually and at time of hire.Pre-hire and annual employee health screening required.Annual influenza vaccine required. Only exception is for employees with a medical or religious exemption approved by Administration. Employees with an approved medical or religious exemption must wear a mask at all times during the flu season.Valid driver's license and access to a reliable car.
