Medical Director Services PC is looking to hire experienced Nurse Practitioners (NP) to service a skilled nursing facility in Syracuse . Provider Duties Serves the role of primary care provider to long and short term residents, assessing patient health by visiting patients, performing physical examinations, and obtaining, updating, and studying medical histories. Team-based care, collaborating with the medical attending/physician and nursing home staff to provide the highest standard of care attainable. Builds relationships with residents and families, educating and formulating a plan of evidence-based care in line with the patients’ needs and desires. Complies with federal, state, and local legal and professional requirements by studying existing and new legislation, anticipating future legislation, enforcing adherence, and advising management on needed actions. Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies. Maintains a safe and clean working environment by complying with procedures, rules, and regulations, ensuring standards align with the core values of the organization: respect, professionalism, and cognizance of cultural, personal, and religious beliefs. Reviews and ensures charting, documentation, and administration of care are up to date and performed in a timely and efficient manner. Requirements Health Promotion and Maintenance, Thoroughness, Clinical Skills, Informing Others, Medical Teamwork, Bedside Manner, Infection Control, Administration of Medication, Pain Management, Self-Development. MDS PC emphasizes quality and customer service first, focuses on treating in place when possible, and reducing unnecessary hospitalizations. We are experienced in providing medical services in an SNF setting, understanding how to give effective medical care and the survey process. We currently provide services in facilities with specialty units and appropriate medical personnel to treat residents on ventilators. It is standard for our providers to reach out to surrounding hospitals to ensure good communication regarding the continuation of care.
