What You Will Do Promote/provide Occupational Therapy services in the home to assist the patient in achieving his/her optimal level of functioning. Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic, and medical conditions. Teach patients/caregivers to become independent with their treatments and self‑management of their chronic illness(es). Coordinate an interdisciplinary plan of care with nursing, physical/occupational/speech therapists, medical social workers, aides, and dieticians. Work hours are primarily Monday‑Friday, generally 8am to 4pm, with some flexibility in schedule. Minimal weekend and holiday rotations. What You Will Need NYS Occupational Therapist Licensure 1 year Occupational Therapy experience required Home care experience preferred but not required Valid Driver’s License and Reliable Vehicle; mileage reimbursement is provided. Care for one patient at a time. Be on the front‑line of the transitions in health care from inpatient settings to the community – home health care is the future! Work schedule is flexible. Eddy VNRA provides a comprehensive orientation program and a supportive work environment including in‑depth computer training, assistance obtaining insurance authorizations, and a Rehab Supervisor only a phone call away. Pay Range $69,907–89,622. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
