Seven Healthcare are seeking an experienced Physical Therapist for a contract assignment in Greenville, AL. This role involves providing physical therapy services in a home health setting, ensuring patients achieve their maximum level of independence and function.

- Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for current diagnosis, past medical history, precautions, and contraindications
- Establish measurable goals and develop treatment plans
- Regularly re-assess the effectiveness of treatment plans and modify as needed
- Communicate patient needs and progress to the treatment team and relevant parties
- Consult with medical providers regarding evaluations, progress, and treatment discharge
- Recommend and assist in securing special adaptive equipment for patients
- Maintain and submit required documentation in compliance with regulations
- Supervise Physical Therapist Assistants and Aides in accordance with state practice act
- Participate in and conduct staff development programs and maintain clinical practice skills
- Valid Physical Therapist license
- Experience in Home Health required
- HCHB experience preferred but not required
