Key Highlights Weekly pay: $2,205–$2,349 per week Guaranteed hours: 35 hours per week Contract duration: 13 weeks Shift: 5x8 days, 08:00–16:30 Start date: August 31, 2026 Setting: Skilled nursing facility Responsibilities Conduct initial evaluations and ongoing assessments to determine PT needs for residents Develop and implement individualized treatment plans, including gait, balance, strength, and mobility training Administer therapeutic interventions and monitor patient response; adjust plans as needed Document evaluations, progress notes, and discharge plans to support regulatory and care-team requirements Collaborate with an interdisciplinary team (nursing, OT, speech therapy, social work) to optimize resident outcomes Educate residents and families on safe-occupational strategies, home exercise programs, and fall prevention Requirements Active physical therapy license in New Hampshire (or eligibility to practice in NH) Graduation from an accredited physical therapy program (DPT preferred) Current Basic Life Support (BLS) certification Experience or strong interest in skilled nursing facility or geriatric rehabilitation Strong communication skills and ability to work in an interdisciplinary team Able to work on-site in Milford, NH and participate in a 35-hour weekly schedule Benefits and Support Guaranteed hours of 35 per week Weekly pay within a set range 24/7 recruiter and travel support throughout the assignment Credentialing and onboarding assistance to facilitate licensure and placement Dedicated on-assignment support to address clinical or logistical needs Find more opportunities If this role isn’t a fit, you can explore other current openings with Healthforce Healthcare LLC.
