Position Description You will serve as the primary care and rounding provider inside one skilled nursing facility in Hopedale, OH, managing a consistent panel of long term care and short stay rehab residents rather than traveling between buildings. This is a full time, in person position, Monday through Friday, 9 to 5, with no nights, no weekends and no on-call. You are employed by our provider network rather than the facility, so you do not report to the DON or the administrator, though you take part in patient care meetings. Qualifications Active Ohio APRN license and DEA registration Board certification as an FNP, AGNP or AGACNP Experience in skilled nursing, long term care or geriatric primary care preferred Comfortable managing a full resident panel independently Responsibilities Provide primary care and rounding for residents in a skilled nursing facility Manage chronic conditions, acute changes in condition and transitions of care Order and interpret labs and diagnostics and adjust treatment plans Document in the facility EMR and participate in patient care meetings #J-18808-Ljbffr
