Community Outreach & Referral Liaison

Charter Healthcare is seeking a Community Relations Liaison to promote our hospice and home health services within designated territories in the United States. You will educate patients, families, and facility staff about our care plans and coordinate with the admissions team to facilitate referrals and admissions.
