Resident Care Coordinator

Position Summary The Resident Care Coordinator is responsible for overseeing the daily delivery of resident care services in the adult home. This position ensures residents receive compassionate, individualized care that promotes dignity, independence, and quality of life. The Resident Care Coordinator supervises direct care staff, coordinates resident assessments and service plans, ensures compliance with state regulations, and serves as a liaison between residents, families, healthcare providers, and the interdisciplinary team. Essential Duties and Responsibilities Coordinate and oversee the daily operations of resident care services. Conduct resident assessments upon admission and at regular intervals to determine care needs. Develop, implement, and update individualized service plans in collaboration with physicians, families, and other healthcare professionals. Supervise, mentor, and evaluate Resident Care Aides, Medication Aides, and other direct care staff. Ensure adequate staffing levels and assist with scheduling as needed. Monitor residents for changes in physical, mental, or emotional condition and communicate changes to the appropriate healthcare providers. Ensure medications are administered according to physician orders and facility policies (if applicable under state regulations). Investigate and document resident incidents, accidents, and concerns. Maintain accurate resident records, care documentation, and regulatory compliance. Promote resident rights, dignity, privacy, and independence. Collaborate with dietary, housekeeping, maintenance and activities to ensure comprehensive resident care. Respond appropriately to emergencies and assist with crisis intervention. Ensure compliance with state licensing requirements, infection control standards, and company policies. Qualifications High school diploma or GED required. Minimum of 2–3 years of experience in assisted living, adult care, long-term care, or a related healthcare setting desired Previous supervisory or leadership experience preferred. Current First Aid certification (or ability to obtain upon hire). Knowledge of state regulations governing adult homes or assisted living facilities. Strong organizational, leadership, and communication skills. Ability to maintain confidentiality and professionalism. Knowledge, Skills, and Abilities Excellent leadership and team-building abilities. Strong assessment and problem-solving skills. Effective verbal and written communication. Ability to manage multiple priorities in a fast-paced environment. Knowledge of infection prevention and resident safety practices. Proficiency with electronic health records and Microsoft Office applications. Ability to work collaboratively with residents, families, physicians, and interdisciplinary teams. Physical Requirements Ability to stand, walk, bend, and lift up to 50 pounds. Ability to assist residents with mobility and transfers when necessary. Ability to respond quickly during emergencies. Manual dexterity sufficient to operate office equipment and electronic documentation systems. Working Conditions Work is performed in an adult home or residential care environment. Exposure to infectious diseases and bodily fluids may occur; appropriate personal protective equipment (PPE) is provided. Requires occasional evening, weekend, holiday, and on-call responsibilities.
