What You’ll Need to Apply:

• Valid HHA or PCA Certification
• Valid Photo ID – Driver’s License, State ID, or Passport 
• Social Security Card
• Permanent Resident Card or Employment Authorization Card – if applicable
• Two (2) Professional References – please have their contact information ready, including name, phone number, and dates employed .
• Pre-Employment Physical – must include: 
  • MMR immunity documentation 
  • TB screening 
  • Drug screening
