Basic QA duties for all program areas include conducting home/community visit observations to comprehensively assess the social workers unmet training needs, maintaining current and accurate documentation of all QA activities in case files and designated databases according to program policies/applicable state and federal laws, educating the public and providers about program services, assisting supervisor with staffing concerns related to QA and program updates needed, overseeing the quality assurance (QA) review process, addressing team education/training needs, participating in special work groups, and coordinating program activities and resources for assigned team. Reviews quality measures standards, identifies deficiencies in service delivery methods and provides recommendations as needed. Regularly travels to field offices to fulfill work duties. Will document and report all relevant workload into the Qualtrics system. Assists program supervisor and manager with coordination and development of policy implementation across programs for services to Children with Special Health Care Needs (CSHCN‑SP), Newborn Screening (NBS), Case Management for Children and Pregnant Women (CPW), Personal Care Services (PCS), and Community First Choice (CFC). Works under limited supervision, with considerable latitude for the use of initiative and independent judgment. Performs other duties as assigned. Member of the Incident Command Structure (ICS) in response to public health emergencies. Must comply with HHS, DSHS, and Texas Department of Public Safety applicable policies/procedures/standards. Must be able to independently travel throughout the State of Texas and out-of-state to complete essential job functions. Essential Job Functions Quality Assurance: Conducts quality assurance activities to assure contract compliance and effective resource utilization with contracted entities. Works with staff to provide internal quality assurance reviews to ensure quality services to clients and monitor program effectiveness in Region 8. Gathers information and prepares corrective action/monitoring plans as needed. Reviews quality measures standards, identifies deficiencies in service delivery methods and provides recommendations as needed. Conducts QA meetings for staff and one to one as needed. Identifies deficiencies and provides feedback to program supervisors on a regular basis. Ensures that case managers are properly documenting all activities in the appropriate database as determined by central office or regional needs. Runs and maintains ongoing reports to ensure staff compliance. Program & Client Services: Serves as Case Management Coordinator for the SHSS staff as well as providing case management support to the program population for PCS, CFC, CSHCN‑SP, NBS, and CPW programs. Duties include assisting with identifying, planning, and coordinating training for staff and providers. Will serve as a mentor and trainer to new Case management staff when directed. Maintains case files Provides Specialized Social Work consultation and assessment to families of children who are medically fragile and other identified populations to determine Personal Care Services Eligibility. Conducts assessments for PCS/CFC and Birth Defects cases. Community Organization: Provide program presentations to community, agencies and health entities. Participate in the public health responses to disaster and emergencies. Participates as a team member for the Department of Social Work Emergency Response Team with other DSHS programs in Region 8. Responds to public health emergencies, including, but not limited to, disease outbreaks, biological, environmental and weather emergencies. Participates as requested in the event of a disaster response and/or recovery effort or Continuity of Operations (COOP) activation. Completes required training necessary to fulfill the duties of the role. Complies with DSHS immunization policy. Maintains an acceptable driving record that meets the standards developed by the Texas Department of Public Safety (DPS) and safely operates either a state-owned, personal or rental vehicle to conduct state business. Maintains a valid Class C Texas driver license or equivalent from another state. Performs other duties as assigned. Knowledge, Skills, and Abilities Knowledge of Case Management principles and practices. Knowledge of children’s programs, such as Medicaid, CSHCN, Case Management for Children & Pregnant Women, CPS/Foster Care, SSI, NBS, NBHS, Lead, Early Childhood Intervention (ECI), and CHIP. Knowledge of quality assurance and technical assistance policies and procedures. Knowledge of community resources on a local, state and national level. Knowledge of the cultural and socioeconomic issues related to health and disabilities. Skill in using computer software programs in word processing, spreadsheets, databases, electronic mail and Internet, specifically MS Word, PowerPoint, Excel, Access and Outlook. Skill in compiling monthly reports, travel reports, and time activity reports. Skill in conducting in-home family needs assessments and developing service plans. Skill in providing presentations and trainings. Skill in conflict resolution, mediation, and problem solving. Ability to work effectively as a team member in an interdisciplinary setting. Ability to interpret and apply program policies and procedures. Ability to provide professional leadership. Ability to maintain confidentiality. Registrations, Licensure Requirements or Certifications Current license to practice Social Work in the State of Texas Initial Screening Criteria Experience in Case Management principles and practices - 2 years preferred Experience in developing and conducting training - 2 years preferred Experience in conducting Quality Assurance activities - 2 years required Experience in the use of computers and related equipment, including the use of Office Suite, spreadsheets, databases and presentation software required - 2 years preferred Experience in written communications as observed by detail and completeness of information on the state application.
