Adoption Program Case Manager

Job Details Location: Gainesville, FL 32653. Education Level: 4 Year Degree. Salary Range: $22.49 – $24.04 hourly. ESSENTIAL JOB FUNCTIONS Provides direct case work pre‑requisite to adoption for adoption goal children and families adopting children, including adoption home studies, child studies, referrals for psychological evaluation, subsidy requests, enhanced subsidy requests, and maintenance of adoption tab information and provider information in FSFN. Completes quarterly home visits for secondary assignments and monthly home visits for primary assignments, ensuring documentation is entered into FSFN within 48 hours of the visit. Maintains up‑to‑date information for all assigned recruited children on the Explore Adoption website, including current photos and web memos submitted for timely posting following TPR. Provides Adoption Case Activity Reports for judicial reviews on secondary assignments; prepares judicial review reports in FSFN on primary assignments addressing child(ren)’s safety, well‑being and permanency. Attends all court hearings and testifies regarding progress, barriers and/or concerns to achieve permanency. Completes all documentation and reports in a legally compliant and timely manner. For primary assignments, attends all psychotropic medication management appointments, obtains completed medical reports including 5339, and submits to CLS to obtain required court orders. Monitors compliance with psychotropic medication rules, completes monthly reviews, obtains necessary documentation, and uploads all medication‑related documents in FSFN. Provides adoptive placement services to adoptive families, documenting in compliance with legal requirements for full disclosure and adoption subsidy. Handles adoption disruptions: documents disruptions, ensures subsidy termination if applicable, and coordinates subsequent placement and family support. Performs specialized case management activities to support program compliance with legislative mandates, DCF and/or PSF initiatives, including the Adoption Incentive Program and local activities promoting timely adoption of waiting children in Circuits 3 and 8. Plans, facilitates, and documents separation of sibling and match staffing. Assesses child and family needs and coordinates delivery of services specific to adoption preparation, transition, and support, including development, documentation, and fulfillment of transition plans for all adoptive placements. Provides lifebooks to adoptive families. Provides on‑call support when needed. Completes incident reports as required. Serves as the primary point of contact to adoptive families with recruited children or other matched children during the 90‑day post‑placement supervision period. Specialized recruitment adoption case managers plan child‑specific recruitment efforts, assist in targeted recruitment by PSF, and participate in match staffing reviews of all available children without identified families every 60 days as required by FAC. Responds timely to inquiries on assigned children and plans match staffing upon identification of appropriate inquiries. Collaborates with Diligent Search Specialist toward family‑finding efforts for waiting children. Maintains the Heart Gallery and promotes general, targeted, and child‑specific recruitment efforts. Displays understanding and sensitivity to the cultural and socioeconomic characteristics of the service population. Participates in annual National Adoption Month events in November. Performs other related job duties as assigned. Qualifications Education and Experience: Bachelor’s Degree in Social Work or comparable human services field; Master’s Degree preferred. Two (2) years of experience in the field of adoption preferred. Adoption Competency training preferred. Special Qualifications: Certification as a Child Welfare Professional (CWP) eligible; must obtain CWP certification within twelve (12) months of appointment. Possession of a valid, State of Florida driver’s license required. Knowledge: Family safety and preservation. Adoption laws and regulations in Florida. Theories and practice of child protection, counseling, social work, investigations, and family assessments. Methods of compiling, organizing, and analyzing data. Department operations, policies, and procedures. Modern office standards, policies, and procedures. Skills: Proficient use of computers and software applications related to essential functions. Effective communication, both orally and in writing. Use of various office equipment (copier, fax, multi‑line telephone). Problem‑solving and analysis. Crisis intervention principles. Case staffing and meeting facilitation. Abilities: Initiation of proactive and decisive action. Accurate assessment of children and families. Travel. Identification of program improvements and adjustments. Supervision of direct staff and development of effective practices. Planning, organizing, and coordinating work assignments. Interacting appropriately with families, community resources, service providers, and other agency professionals. Work some evenings and weekends conducting PRIDE trainings. Maintain a positive and reliable attitude toward a challenging environment. Sensitivity to cultural needs and willingness to serve as a positive team member. Prioritization of multiple tasks and projects. Independent work with minimal supervision. Establish and maintain effective working relationships with other personnel and the public. #J-18808-Ljbffr
