Apply your expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care. + Craft individualized care. Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being. + Collaborate and coordinate. Work seamlessly with a multidisciplinary team - physicians, therapists, and support staff - to ensure holistic care. + Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support. + Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team. + Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation. **Job Requirements** **This is a PRN Position** Qualifications: + Graduate of an accredited nursing program with an active RN license in the state served. + Minimum of 1 year of nursing experience (Home Health experience is a significant plus!). + Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care. + Current CPR/BLS certification. + Valid driver's license and reliable transportation.
