Shift: 2nd shift 6:00pm-6:00am

Job Description

• Be responsible for the care and well-being of the residents in your charge

• Assess the residents’ needs and assist in developing and following individual care plans

• Assure quality care is delivered through the utilization of nursing processes and guidelines

• Identify and communicate changes in residents’ status to physicians, staff, and family members

• Administer medications, treatments, and other direct care as prescribed and indicated

• Coordinate admissions, transfers, and discharges

• Communicate appropriate information to all staff to provide a smooth shift transition

• Investigate and document incidents and accidents as required

• Document appropriately as needed or required in PCC.

·  Competitive salary RN $40-$42 an hour/ LPN $34-$36 an hour

1st shift differential .50 an hour

2nd shift differential $1.00 an hour

Weekend differential $2.00 an hour

• $2,500 sign on bonus paid at $500 every 3 months until paid in full

Qualification

• Current Indiana RN or LPN License

• Current CPR certification

• Experience working in a treatment center
