Licensed Practical Nurse (LPN) - Long Term Care | NightsLocation: Springfield, CO Facility: Southeast Colorado Hospital District Job Type: 13-Week Contract Start Date: July 7, 2026 Schedule: 36 Hours per Week | Night Shift (6:00 PM - 6:00 AM) Position Overview Southeast Colorado Hospital District is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join its Long Term Care team for a 13-week contract assignment. The LPN is responsible for providing direct and indirect nursing care to residents while ensuring high-quality, patient-centered care in a collaborative environment. This role includes supervising Certified Nursing Assistants (CNAs), administering medications, coordinating resident care, and maintaining accurate clinical documentation. Key Responsibilities Provide comprehensive nursing care to residents during assigned shifts. Assign, direct, and supervise resident care provided by CNAs and support staff. Administer medications and treatments as prescribed by physicians. Monitor residents' conditions and promptly report significant changes to nursing leadership. Document resident assessments, acute illnesses, physician visits, passes, and other clinical updates accurately and in a timely manner. Communicate resident status, care updates, and new physician orders with families and designated medical power of attorney (MDPOA) as appropriate. Collaborate with physicians, nursing leadership, and interdisciplinary teams to ensure continuity of care. Promote teamwork and effective communication across departments and shifts. Participate in staff meetings, education sessions, and ongoing training. Perform additional duties as assigned to support quality resident care. Qualifications Active Licensed Practical Nurse (LPN) license. Current BLS (Basic Life Support) certification. Minimum of one year of nursing experience. Strong clinical assessment, communication, and organizational skills. Long Term Care experience is preferred.
