Nurse Practitioner

We are seeking a Primary Care Provider (NP, FNP, or ANP) with experience or a strong interest in HIV care to join a collaborative, multidisciplinary team focused on delivering comprehensive primary care services in an affirming and patient-centered environment. What You'll Do Provide comprehensive primary care services, including patient assessments, diagnosis, treatment, and preventive care. Manage and support patients living with HIV while promoting overall health and wellness. Deliver evidence-based, culturally competent care to a diverse patient population. Coordinate care with specialists, behavioral health providers, case managers, and other members of the care team. Review test results, manage follow-up care, and ensure timely documentation in EPIC. Participate in quality improvement initiatives, case conferences, and ongoing professional development opportunities. What We're Looking For Nurse Practitioner with an active New York license. Experience in Primary Care, Family Medicine, Internal Medicine, Community Health, or HIV Medicine. Commitment to providing respectful, compassionate, and inclusive care. Strong communication, collaboration, and patient engagement skills. Experience working with diverse and underserved populations is highly valued. Why Join?
