Field Outreach Specialist: Community Access Advocate

Pacific Health Group

Pacific Health Group is hiring Street Outreach Advisors to connect community members with care and support services in South San Diego County. This field-based role focuses on in-person outreach, building partnerships, and expanding access to enhanced care management and behavioral health services.
