Part-Time Physical Therapist - Flexible, Rewarding Opportunity (Min. 15 Visits/Week)

What You'll Do

Perform comprehensive patient evaluations and develop individualized treatment plans
Deliver hands-on, goal-driven physical therapy to improve mobility, strength, and function
Track patient progress and adjust care plans to maximize outcomes
Educate patients and families to support recovery and long-term success
Maintain timely and accurate clinical documentation
Collaborate with healthcare team members to ensure coordinated, high-quality care

What We're Looking For

Licensed Physical Therapist (PT) in good standing
Strong clinical skills with a patient-first mindset
Excellent communication and documentation abilities
Ability to work independently while contributing to a collaborative team
Commitment to quality care and positive patient outcomes

Requirements

Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver's license
Valid Auto Insurance
HomeCare HomeBase experience preferred

Physical Demands

Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
Must be able to lift, carry, pull, or push up to 100 lbs..
