Your role is instrumental in maintaining high standards of care while navigating the unique challenges associated with remote work in a hospice setting. Key Responsibilities Directly engage with and assess clients who present with major mental health issues or severe emotional disorders. Conduct evidence-based screenings and coordinate necessary follow-up assessments, ensuring personalized care plans are maintained. Develop and implement integrated care plans, facilitating communication among clients, families, and interdisciplinary care teams. Provide therapeutic interventions including individual, group, and family therapy, while addressing each client's unique challenges. Monitor clients regularly, ensuring that their treatment services align with their presenting concerns and personal goals. Participate in on-call coverage to support clients and staff outside of regular program hours. Contribute to enhancing agency service delivery by participating in professional committees and developing best practices. Manage administrative tasks associated with client documentation, ensuring timely submission of reports and adherence to confidentiality standards. Patient Population / Client Focus This role primarily focuses on serving patients within a hospice care environment. Clients referred to our services often face complex emotional challenges stemming from terminal illnesses, grief, and end-of-life issues. As an LCSW, you will not only provide psychological support to these individuals but also engage with their families, fostering a supportive space that promotes healing and acceptance during difficult times. Qualifications A Bachelor’s degree in Social Work or a related field is a minimum requirement; a Master’s degree is preferred. A valid LCSW license in the state of Virginia is mandatory. At least 2 years of clinical experience in a behavioral health setting, preferably with hospice, palliative care, or similar environments. Proven ability to establish rapport with clients and collaborate effectively with interdisciplinary teams. Exceptional communication and interpersonal skills to work with diverse populations. Preferred Experience Experience using telehealth platforms for therapy sessions. Familiarity with integrated care models that encompass both physical and mental health. Background in crisis intervention techniques and emotional support within end-of-life care contexts. Compensation and Benefits The salary for this position ranges from USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
