Case Coordinator

Job Title: Case Manager Team: Programs Classification: Full-Time, Exempt Reports To: Case Management Supervisor Direct Reports: None Effective Date: August 2026 Position Summary The Case Manager is responsible for an assigned caseload of Toby’s Place residents, helping them transition into the program, providing encouragement and accountability, and assisting them in overcoming barriers to stability, recovery, independence, and sustainable transition. The Case Manager develops professional helping relationships with residents and facilitates person-centered assessment, planning, service coordination, resource navigation, advocacy, progress monitoring, and transition planning. The Case Manager works collaboratively with residents, the Care Team, Spiritual Formation, Residential Support, Intake & Program Access, community resources, and other service providers to support individualized goals and coordinated care. The Case Manager will maintain accurate and confidential case documentation, monitor resident progress, identify changing needs and barriers, and ensure appropriate follow-through on referrals and services. The residents remain active participant in defining their goals, priorities, preferences, and desired outcomes. The Case Manager may be required to participate in on-call duties and some evening and weekend work as assigned. Documented experience engaging with and managing at-risk populations, Documented experience and education relating to ministry to women and children, Documented experience in working well with various races, ethnicities, cultures, disabilities and age’s Proven ability to work independently while staying aligned with the priorities of the supervisor Knowledge of PC-based software and relevant applications and office equipment Position Responsibilities Person-Centered Assessment & Planning Build professional helping relationships with residents that demonstrate dignity, respect, compassion, and appropriate choice. Complete assigned assessments and gather information necessary to understand each resident’s strengths, needs, preferences, barriers, resources, risks, and priorities. Facilitate collaborative, person-centered planning in which residents actively participate in defining their goals, priorities, responsibilities, and desired outcomes. Develop and maintain Individual Case Plans with clear, measurable, and achievable goals appropriate to each resident’s needs. Review and update Individual Case Plans according to established timeframes and whenever significant changes in needs, circumstances, progress, or preferences occur. Provide residents with encouragement, accountability, and support as they work toward established goals. Case Load & Resident Support Maintain an assigned caseload of residents, providing case coordination meetings on a weekly and/or as-needed basis. Oversee all intake processes for assigned residents. Manage workload so that urgent needs, significant barriers, required reviews, referrals, documentation, and transitions receive timely attention. Monitor resident progress and identify achievements, stalled progress, emerging barriers, and changing needs. Communicate promptly with the Lead Case Manager when case complexity, caseload volume, safety concerns, or competing demands may affect the quality or timeliness of service. Participate in caseload review and reassignment processes as needed to support continuity of care. Support residents in overcoming barriers that may prevent progress toward stability, recovery, independence, and successful transition. Service Coordination, Resource Navigation & Advocacy Connect residents to appropriate community resources and services based on individualized needs and established case plans. Maintain a working knowledge of relevant community resources, support systems, referral pathways, and services available to residents. Coordinate services that may include housing, employment, financial resources, identification, benefits, healthcare access, transportation, appointments, education, legal/community resources, childcare, and other needs within the scope of the position. Coordinate services between the children’s ministry program, local daycare facilities, and RSD 2, as applicable. Follow up with residents and service providers to determine whether referrals and planned services were accessed and whether additional barriers remain. Advocate appropriately on behalf of residents while maintaining confidentiality, professional boundaries, and role responsibilities. Connect residents with natural, community, and social support when appropriate to their goals and preferences. Care Team & Interdisciplinary Collaboration Work collaboratively with the Care Team to promote coordinated services and positive resident outcomes. Participate actively in Care Team meetings and communicate information necessary for coordinated care while maintaining confidentiality. Communicate significant resident concerns, changes, barriers, and progress to appropriate Care Team members. Recognize when another department, professional, or service provider should take the lead and coordinate appropriately rather than duplicate services. Complete assigned follow-up from Care Team decisions and document responsibilities and outcomes. Work collaboratively with Spiritual Formation, Residential Support, Intake & Program Access, and other appropriate Oliver Gospel personnel. Participate in ongoing evaluative meetings with the supervisor. Mental & Behavioral Health Coordination Support compliance with treatment plans established by qualified medical and/or mental and behavioral health professionals for assigned residents, within the scope of the Case Manager role. Coordinate with external providers through established referral, consent, communication, and confidentiality procedures. Respect the professional scope of medical and behavioral health providers and do not diagnose, treat, or provide clinical services beyond the Case Manager’s qualifications and authorized role. Communicate relevant concerns to the appropriate Care Team member or supervisor when additional professional support may be needed. Program Compliance & Resident Accountability Ensure adherence to program guidelines and operational and security procedures. Conduct regular Case Coordination meetings and oversee assigned phase transitions. Work with the Care Team to assess resident readiness for phase transitions. Support transition and discharge planning from the beginning of service as appropriate. Coordinate appropriate handoffs and continuity of services when residents transition between programs or into community-based support. Teach program classes as necessary. Provide appropriate accountability while maintaining a professional, respectful, and supportive relationship with residents. Documentation, Quality & Accountability Maintain updated, timely, accurate, objective, and confidential case records for assigned residents. Document assessments, Individual Case Plans, resident meetings, referrals, services, progress, barriers, significant changes, Care Team follow-up, phase transitions, and other required information. Protect resident information and follow all applicable confidentiality, consent, record management, and information-sharing requirements. Submit accurate and timely reports to the supervisor. Participate in documentation review, supervision, training, and quality-improvement activities. Correct documentation or practice deficiencies promptly when identified. Attend and actively participate in meetings as assigned by the supervisor. Professional Development & Continuous Improvement Maintain professional boundaries and exercise sound judgment when working with complex or emotionally charged situations. Participate in required training and ongoing professional development relevant to the population served and responsibilities of the position. Seek guidance and consultation when resident needs exceed the Case Manager’s experience, authority, or role. Receive and implement coaching and feedback from supervisors. Contribute observations and recommendations that may improve case coordination, resident experience, interdisciplinary collaboration, and program outcomes. Perform other duties as assigned by the supervisor.
