Nurse Practitioner

Per Diem NP/PA, Senior Community Care - Pasco, WA $5,000 Sign on Bonus Optum Home & Community Care (Senior Community Care) is hiring a Per Diem Nurse Practitioner or Physician Assistant to provide high-quality, coordinated care to Medicare and Medicaid patients in nursing homes, assisted living, senior housing, and occasional private residences . In this role, you’ll deliver primary care services , manage chronic and acute conditions , support care transitions, and partner closely with facility staff, families, and physicians to improve outcomes and reduce unnecessary hospitalizations.
