We seek a Value Based Care Primary Care Nurse Practitioner or Physician Assistant to support our growing Saginaw/Bay City, MI team. As a Primary Care Advance Practice Provider you will drive HEDIS and quality-focused patient outcomes with a supportive operations team (MA, Scheduler, Social Worker, Nurse Care Coordinator, Pharmacy, Lab). You will get the build a strong relationship with your own patient panel managing the chronic conditions of high-risk Medicare patients Monday - Friday. This full-time role comes with full-benefits, salary of $140k/yr, quality-based bonus program & $7,500 sign on bonus.
