Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers for individualized care plans
• Identify and reassess cost-efficient, appropriate care levels
• Arrange non-medical support to ensure treatment compliance
• Coordinate with various professionals for patient transition to appropriate care levels
• Engage specialty resources for optimal resolution
• Maintain detailed records of clinical, functional, and fiscal outcomes
• Identify health promotion and illness prevention opportunities
• Prevent and intervene in adverse patient occurrences to minimize poor outcomes
• Advocate for patient privacy and confidentiality

Requirements of the RN - Case Management - Travel

• Associates or Bachelor Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license
• Florida License or compact license accepted
• Ability to analyze complex medical information and make decisions
• Competent in Home Care Home Base/Pointcare and Oasis collection
