Southern Health Partners is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide chronic care services at the Madison County Jail in Alabama.

What You'll Do

As a Chronic Care LPN, you'll play an important role in helping patients manage ongoing health conditions and receive consistent, quality care.

• Provide nursing care to patients with chronic and ongoing healthcare needs
• Monitor patients' conditions and identify changes that may require additional evaluation
• Administer medications and treatments according to provider orders
• Perform nursing assessments and document patient care accurately
• Assist with chronic disease management and follow-up care
• Communicate patient needs and changes in condition to the appropriate healthcare providers
• Educate patients on medications, treatment plans, and healthy practices when appropriate
• Maintain accurate medical records and follow established nursing protocols
• Collaborate with the healthcare team to support continuity and quality of care

Qualifications

• Current, unrestricted LPN license in Alabama
• Current CPR/BLS certification
• Strong clinical assessment, medication administration, and documentation skills
• Ability to work independently while collaborating with the healthcare team
• Compassionate, professional approach to patient care
• Previous correctional, chronic care, or behavioral health experience is helpful but not required
