Community Resource Specialist

Overview Easterseals is hiring for a Community Resource Specialist for our Aging and Disability Resource Center. The role provides unbiased and person-centered education, advocacy, assistance and support to NH residents to help maintain dignity, quality of life and independence. The position supports individuals in making informed choices regarding long-term services and navigating community resources. Schedule: Full time 40 hours per week; Monday-Friday 8a-4:30p or 8:30a-5p. Location: Willingness to work between the Manchester and Nashua offices. Compensation: $21/hour. Responsibilities Perform an assessment of the contact/caller’s situation/needs and provide referrals to long-term supports and services through telephone contacts, walk-ins, emails, home visits, community settings and multimedia. Provide information on and referrals to community resources which may meet the needs of the contact/client. Provide long-term support counseling to select individuals in hospitals, rehab facilities, and/or nursing homes. Complete eligibility screening and provide information to assist individuals in understanding and applying for State and Federal programs. Complete documentation on all contacts using the assigned database following all protocols/processes as mandated by the contract and in compliance with InformUSA. Complete follow-ups on contacts following InformUSA protocols and contract requirements. Comply with completion of reporting requirements mandated by the ADRC contract. Provide Medicare information and counseling to Medicare beneficiaries, especially during the Medicare Annual Enrollment Period. Attend meetings as mandated by contract or required by State. Assist with general office support including answering phones, assisting, walk-ins and other office tasks as needed. Obtain all mandatory certifications as required by the contract, including InformUSA (AIRS) and SHIP Counselor within six months to one year of hire. Qualifications Bachelor’s Degree in Social Work, Education, Human Services, Sociology, Cultural Anthropology, or Behavioral Science, OR Eight years’ experience working directly with programs supporting the elderly or disabled population in lieu of degree. Basic knowledge of community long-term care support services for the elderly and disabled population, OR combination of education and experience totaling eight years. Basic knowledge of community services such as home health agencies, assisted living facilities, nursing homes, and hospitals. Basic knowledge of State and Federal programs such as Medicaid and Medicare. Strong oral, written and interpersonal skills. Analytical and problem-solving abilities. Comfortable with performing home visits and community outreaches. Basic level of computer skills including Microsoft Office and database programs.
