Responsibilities Provide care services to individuals with intellectual and developmental disabilities. Provide community integration and inclusion guidance, and consultation to individuals, families and involved persons. Develop opportunities for persons with disabilities to participate in community activities while ensuring the health, safety, and well‑being of residential participants living in the facility through proper supervision. Benefits Professional development and career advancement opportunities Competitive compensation Medical, dental, vision insurance for full‑time employees 401(k) investment plan with company match Generous paid time off and paid holidays for full‑time employees Tuition reimbursement for full‑time employees The opportunity to help us make a difference in the lives of the patients and communities we serve Qualifications High School diploma or GED Must be 21 years of age or older Possess valid driver’s license with acceptable driving record, MVD report and reliable transportation Ability to prepare required paperwork to support and document activities in the contractually required timelines Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping Ability to successfully complete certification and training for First Aid, CPR, Article 9, Medication, Prevention/Support, Direct Care Worker and any other required training; the agency will provide training if the candidate does not possess Able to operate independently with minimal oversight Demonstrated skills to ensure respectful communication to participants and coworkers Must obtain and maintain a fingerprint clearance card and pass a background check along with Central Registry Clearance Willingness to work nights, weekends, holidays and a varied schedule each week Preferred Qualifications Bilingual English/Spanish or English/Sign Language Prior experience in habilitation or rehabilitation Two years of experience in a person‑centered community integration program assisting persons with developmental and/or psychiatric disabilities Two years of experience in habilitation/rehabilitative assistance to persons with psychiatric and/or developmental disabilities Very strong interpersonal skills #J-18808-Ljbffr Horizon Health And Wellness
