Position requires travel in the community. The majority of our patients prefer to receive hospice and palliative care at home to maintain their independence and live their lives to the best of their ability. Symptoms are managed so that the patient can live a full life with the support of family and friends and the comforts of home. Spiritual Care Counselors provide guidance for the spiritual and emotional needs of patients, families, and staff. They participate in and conduct worship, including various ceremonies/rituals, during visits to new patients and follow‑up visits, as requested. They work as part of an interdisciplinary team of care givers within the HBI/CHPC network. Essential Duties and Responsibilities Assess and provide for patient spiritual care needs, as well as those of primary caregivers and other family members as appropriate in compliance with all organizational, regulatory, and professional standards. Build rapport with patients and family members, utilizing effective communication skills with openness toward different lifestyles, cultures, beliefs, and values, and understand the value of being a non‑judgmental, listening presence. Collaborate with interdisciplinary groups to arrange and successfully implement patient‑driven care plans, with particular responsibility for spiritual care needs. Adhere to the requirements of both CHPC and Hospice Buffalo policies and procedures, which are patient‑focused (clinical) and organization‑focused (administrative). Adhere to HIPAA regulations and policies. Effectively and accurately document ongoing patient information within the provided electronic medical record, according to organizational and professional standards, and maintain their security. Document to agency standards inclusive of timely completion, timely signatures, and documents care using the A‑I‑R protocol. Participate in ongoing education and professional development to maintain and acquire current expertise in the field of spiritual end‑of‑life care. Actively participate in team meetings, committees, and quality improvement initiatives as necessary. Serve as a spiritual care resource and presence to Hospice staff and volunteers as related to patient care issues. Facilitate involvement with local communities as necessary. Participate in the organization’s memorial services, speaking events, and gatherings as needed. Qualifications Education/Certification: Master of Divinity, Pastoral Studies, Religion, or Theology, or comparable field preferred. Successful completion of one or more units of Clinical Pastoral Education (CPE) in coordination with management. Board‑certified Chaplain (or eligible for board certification) status preferred. Valid driver’s license, free of serious violations; vehicle required for use when on duty or on‑call. Experience: Two (2) years of experienceworking in spiritual or pastoral care, specifically with terminally ill patients. Salary: $26.00 per hour. #J-18808-Ljbffr
