Bilingual Care Manager — Remote Patient Engagement Pro
Vigilance Health, Inc.
Vigilance Health, Inc. is seeking a Care Manager / Patient Engagement Specialist to build trust and guide patients on their health journeys. This remote, full-time role emphasizes high-volume outreach, Motivational Interviewing, and strong documentation across multiple EHRs.
You will coordinate care plans and engage patients to improve health outcomes. Ideal candidates bring RN/LVN/LPN or MSW/LCSW backgrounds and bilingual communication skills.
