Job Title: Contingent Clinical Care LVN/LPN PT - MC Address: Pomona, CA Job Description Administer oral, injectable, intravenous, and topical medications. Evaluate complex patient conditions, monitor pain levels, and document treatment responses. Apply advanced knowledge of vital sign monitoring to identify and record abnormalities. Perform routine assessments to monitor patient health and ensure overall well-being. Manage patient access to healthcare and connect individuals with community resources. Accurately document patient medical history using electronic health records. Critical Information This is a temporary/contract position. Work schedules will be created in 6-week blocks following training/orientation. Shifts may be between 7-10 hours during clinic operating hours. Availability to work two or three weekdays and every other weekend is required. This position requires working on some major holidays. Education/Licenses Needed Full, active and unrestricted license as a Licensed Practical or Vocational Nurse in a State, Territory or Commonwealth of the United States or District of Columbia. Minimum of one-year related work experience in a healthcare setting. Proficiency with electronic medical record (EMR) systems; Epic experience preferred.
