In this full-time remote role, you will play a vital part in providing comprehensive psychosocial services to patients and their families within the supportive environment of our Hospice Center. This position offers a competitive annual salary range of $120,000 to $130,000, commensurate with experience and qualifications.

Key Responsibilities\n

• Psychosocial Assessment: Conduct comprehensive psychosocial assessments in a timely manner, applying knowledge of human behavior, psychosocial factors influencing treatment outcomes, and relevant ethical considerations.

• Clinical Intervention: Deliver supportive counseling for patients and families, facilitate family meetings concerning difficult decisions such as hospice care, Do Not Resuscitate (DNR) plans, and placement, while assisting families in articulating their concerns effectively to the healthcare team.

• Case Management: Utilize effective triage skills to manage clinical caseloads, identify high-risk situations, and balance referrals and patient care needs while collaborating with Case Managers on discharge planning.
\n
• Legal and Ethical Compliance: Ensure all clinical assessments and interventions adhere to legal and ethical standards, including responding appropriately to issues of suspected abuse or neglect and maintaining patient rights.
\n
• Documentation: Ensure all documentation complies with established policies, including interdisciplinary care plans and clinical progress notes, to maintain accurate patient records.
\n
• Quality Improvement: Actively participate in quality improvement initiatives, monitoring, and analyzing patient care indicators to enhance service delivery within the organization.

• Training and Development: Provide in-service training sessions for hospital staff focusing on psychosocial aspects of patient care, fostering a collaborative learning environment.

• Other Duties: Engage in special projects, support groups, and educational programs aimed at improving the quality of life for patients and their families.

Patient Population / Client Focus\n

You will primarily serve patients in hospice care, a sensitive and crucial demographic requiring tailored support. Your role will entail working closely with patients facing terminal illnesses, their families, and interdisciplinary healthcare teams to navigate complex emotional and practical challenges during this critical time of life.

Qualifications\n

• Master's Degree in Social Work from an accredited graduate program.

• Minimum of two years of experience in a medical setting, with at least one year in a hospice or cancer care context.
\n
• Strong verbal and written communication skills in English, along with effective interpersonal skills to build rapport with patients and team members.

• Demonstrated commitment to providing exceptional customer service and patient care.
\n
• Analytical and problem-solving abilities aligned with the complexities of patient needs.
\n
• Flexible and collaborative, capable of functioning both independently and as part of a multi-disciplinary team.

Preferred Experience\n

• Previous experience supervising Master’s level social work students and staff would be an asset.
\n
• Knowledge of community resources and referral processes in the Pomona area.
\n
• Demonstrated experience in developing and facilitating psychosocial support programs.
