Actify Home Care

Actify Home Care in Boca Raton seeks licensed Physical Therapists for immediate PT home health assignments. This 1099 role offers per-visit compensation with quick onboarding and credentialing to get you started rapidly. You'll provide one-on-one PT in geriatric home settings, develop plans of care, document evaluations and re-assessments, and coordinate with our clinical team to ensure high-quality patient outcomes.
