Behavior Support Specialist (BSS) – Lewistown, PAFull-Time | Salary: $65,000-68,000Join a team that believes behavior is communication and that meaningful change happens through relationships, not control.Person Driven Clinical Solutions (PDCS) is seeking a compassionate, skilled Behavior Support Specialist (BSS) to provide person-centered behavioral support services to individuals with intellectual and developmental disabilities throughout the Philadelphia area. The BSS partners with individuals, families, providers, and interdisciplinary teams to understand needs, build strengths, reduce barriers, and improve quality of life.In this role, you will conduct assessments, develop behavior support plans, provide consultation and training, analyze data, attend team meetings, and help create environments where people can thrive.We are looking for professionals who value collaboration, trauma-responsive practices, positive approaches, and helping people achieve meaningful outcomes in their everyday lives.What You'll DoComplete evaluation / detailed assessment of needs for individuals needing a Behavioral Support Plan (BSP) or a Restrictive Procedure Plan (RPP)Collect and evaluate behavioral dataObserve individual in various settings for the purpose of developing a BSP/RPPCollaborate with the individual, their family, and their team for the purpose of developing a BSP/RPPConducting comprehensive functional assessments of presenting issues (e.g., aggression, self-injurious behavior, law offending behavior, elopement, etc.)Partner with the individual, their family, and their team in order to develop positive interventions to address specific presenting issuesDevelop individualized BSP/RPPEnsure that all support plans are Person Driven, integrate Practice Based Evidence and reflect the core clinical needs of the individualTrain and mentor support/program staff, families and team members on the individual's behavior support plan and the individual preference for supportsImplement activities and strategies identified in the individual's BSP/RPPMonitor implementation of BSP/RPPCollect, organize, review, and analyze data on challenging strategiesMaintain and update individuals' behavioral data tracking tools/create progress documentation utilizing excel and convey trends, progress, successor the need to change approachesReview and revise BSP/RPP annually foreach individual at Individual Support Plan (ISP) Annual Review or whenever a Critical Revision (CR) is requested by the individual's supports teamCollect BSP/RPP signatures electronically through DocuSignPartner with interdisciplinary teams to schedule and facilitate annual training on BSP/RPPAttend (as required/ virtually) psychiatric, medical, and therapy appointments as needed for consumersParticipate as needed in Interdisciplinary Team and Quarterly ISP meetingsEnsure /Protect all individual's human rightsParticipate in ongoing Human Rights Team (HRT) ReviewsComplete the individual(s) HRT Summary and necessary supporting documentationParticipate in various County Human Rights Committee (HRC) ReviewsComplete the necessary supporting documentation and coordinate with Regional Director that all necessary documentation is complete/accurateParticipate in various County Positive Practices Review Committee (PPRC) upon requestCollects feedback on the individual and team members' experiences of the supports and services providedComplete accurately documentation of billable/non-billable time in SETWorks, Progress Notes (PN) and Monthly Progress Review Summary Notes (MPRSN) daily/weekly/monthly for every individual on BSS caseloadSubmit required monthly oversight information to supervisorsParticipate in necessary supervision/mentoring meeting with supervisorReview ISP Caseload Tool monthly for accuracyReview Authorization Spreadsheets and update the team on any changes in needTimely submit proper, complete and accurate required documentation/ paperwork as required by State and Federal Regulations, PDCS, and HIPAAComplete PDCS Annual Training requirementsFollow up on correspondence/mailings/filing as neededWho We're Looking ForYou may be a great fit if you are:Compassionate and caringDependable and responsiblePatient and positiveLooking for meaningful workInterested in healthcare, human services, psychology, education, nursing, social work, or caregivingReady to make a difference in the lives of othersQualificationsBachelor's degree in a human services field required; master's degree preferredExperience working with individuals with intellectual and developmental disabilities, autism, mental health needs, or behavioral challengesKnowledge of person-centered and positive behavioral support practices, trauma-responsive supports, and collaborative service deliveryExcellent communication skillsExcellent writing, documentation, and record keeping skills; particular emphasis on Functional Behavioral Assessments (FBA) and written behavioral/treatment plansStart a Career with PurposeSome jobs pay the bills.This job changes lives.If you're looking for a career where your compassion, dedication, and hard work make a real difference every day, we invite you to join the Person Driven Clinical Solutions team.Apply Today and Become the Reason Someone Has a Better Day.Person Driven Clinical Solutions is proud to be an Equal Opportunity Employer committed to building a diverse and inclusive workforce."Creating a Better Tomorrow...
