Description:
The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on their assigned unit.
The individual must demonstrate knowledge of the principles of growth and development across the lifespan.
They must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs.
The LPN will provide direct and indirect skilled nursing care for assigned residents.
The LPN will follow defined policies and procedures under the supervision of a Registered Nurse.
They will maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
The LPN must maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
The LPN shall have the ability to provide care to geriatric residents.


Job Duties:
The LPN collects data relevant to the resident's physical and psychosocial condition on an ongoing basis.
They will communicate findings to the Registered Nurse and other members of the Interdisciplinary Team.
The LPN contributes to the creation of a developmentally age-appropriate and culturally relevant plan of resident care.
This plan will reflect nursing diagnoses and interventions to achieve desired resident outcomes.
The LPN will involve the resident, family, and healthcare team in this process while promoting a healthy and safe environment.
They will recommend revisions to the plan of care after evaluating resident outcomes.
They will indicate the need for referrals to ensure all aspects of the resident's care have been addressed.
The LPN collaborates with the resident, family, and members of the Interdisciplinary Team within the scope of nursing practice to ensure customer satisfaction.
They must maintain current knowledge and competency in nursing practice.
The LPN participates in the mentoring and orientation of staff.
They will consider factors related to safety, cost, and impact when planning for the delivery of care.
Under the direction of the Registered Nurse, the LPN will provide leadership in the professional practice setting.
They demonstrate knowledge and competency.
The role involves occupational exposure to bloodborne pathogens, with required tasks involving potential contact with blood, body fluids, or tissues.
