Key Highlights Weekly pay: $2,287–$2,377 per week Guaranteed hours: 40 per week Contract length: 13 weeks Shift: 8-hour days, 8x5 Start date: 11/15/2026 Setting: hospital-based hospice care No on-call duties Responsibilities Perform biopsychosocial assessments for hospice patients and families; contribute to individualized care planning Develop and implement care plans in coordination with an interdisciplinary team; support discharge planning and community resource referrals Provide psychosocial support, counseling, and education to patients and families about goals of care, coping strategies, and advance directives Document assessments, plans, interventions, and progress in the Epic EMR; maintain accuracy and HIPAA compliance Assist with safety, mobility, and functional support; lift up to 50 lbs intermittently as clinically indicated Travel to patient locations within the assigned radius; navigate varied road conditions and weather as needed Collaborate with the department lead and other team members to ensure comprehensive, patient-centered care Requirements Active driver’s license and access to a reliable vehicle capable of driving in varied conditions (gravel roads, snow, rain, ice) Active Basic Life Support (BLS) certification Master’s degree in Social Work (MSW); at minimum, working toward Licensed Clinical Social Worker (LCSW) Minimum 2 years of clinical/ED/Medical Social Worker experience Ability to work in a hospice setting with adult patients; majority of patients are 65 years and older Ability to perform lifting up to 50 lbs intermittently Benefits and Support Guaranteed hours: 40 per week Assignment length: 13 weeks Weekly pay: $2,287–$2,377 Shift: 8-hour days, 8x5 Travel assignment with dedicated assignment support PAY DISCLAIMER RULE: Please note: Pay rates and guaranteed hours are estimates and may vary.
