Position Summary: As an Addiction and Mental Health Case Manager, you will meet individually with clients, run group therapy, documentation, and perform crisis intervention as needed. You will monitor clients to evaluate the success of their treatment and adapt treatment as needed. You will prepare a treatment plan for all assigned Clients and update within agency guidelines.

Responsibilities and Duties

Essential Duties and Responsibilities:

• Conduct comprehensive client assessments to identify mental health and substance abuse needs.
• Develop individualized treatment plans in collaboration with clients, ensuring goals are client-centered and achievable.
• Provide case management services, including ongoing monitoring and support, to ensure clients are making progress toward their recovery goals.
• Coordinate with external resources, including social services, healthcare providers, and community organizations, to ensure clients receive necessary services.
• Facilitate therapeutic interventions, including individual and group counseling, as needed.
• Advocate for clients to access appropriate services and resources.
• Document client interactions and progress in accordance with organizational and regulatory standards.
• Maintain confidentiality and uphold ethical standards in all aspects of client care.
• Coordinate efforts with Clinical Director and additional professional staff.
• Attend training sessions and in-services to increase knowledge and skills.
• Participate in all mandatory staff meetings.
• Aid clinical teams in discharge planning and work collaboratively to ensure successful aftercare plans are completed.

Qualifications and Skills

Qualifications:

• High School diploma or GED equivalent
• Prior history of working in a treatment center for substance abuse and mental health
• Preferred Candidates: Experience in treatment related field(s): Social Work, Psychology, Counseling, or a related field and have SUDCC OR CADC-1
• Knowledge of therapeutic techniques, treatment models, and resources for substance abuse and mental health recovery.
• Strong interpersonal, communication, and organizational skills.
• Ability to work effectively as part of a multidisciplinary team.
• Commitment to providing compassionate, client-focused care.
• Ability to maintain professional boundaries while supporting clients in a non-judgmental manner.
• Minimum requirements: Minimum of RADT/ SUDRC/ CADPT and or equivalent credential from an accreditation/ certification body. (with proof of enrollment in an AOD accredited program)
• This position also requires proof of a current negative TB test, CPR/First-Aid certification, and documentation for I-9 verification within the first week of employment.

• Must be well-versed in counseling protocols and techniques, have excellent clinical skills and maintain professionalism in dress, speech and actions.
• Must have good problem-solving skills, work well with others and be a team member.
• Must have expertise in communicating with highly developed written and verbal skills.
• Must have a working knowledge of twelve-step programs and principles.
• Ability to recognize the importance of adapting to various groups.
• Ability to adhere to safety policies and procedures.
• Ability to use good judgment and maintain confidentiality.
• Ability to demonstrate tact, resourcefulness, patience, and dedication.
• Ability to accept direction and adhere to policies and procedures.
• Ability to react calmly and effectively in emergency situations.

 

Job Type: Full-Time (approximately 30-35 hours a week)

Monday-Friday hours include 8:30/9:00 am-4:00/4:30 pm and a few evening hours (hours vary according to census)
