Registered Nurse - New Graduates Welcome

Join a dedicated interdisciplinary team providing community-based nursing care for frail elders. This full-time nursing role involves assessment, care planning, and care coordination to support participants' health and social needs, promoting their ability to live safely in the community. Key Responsibilities Include Conducting initial and ongoing assessments of participants. Developing and updating personalized care plans in collaboration with the team and family. Monitoring health status, identifying urgent needs, and ensuring tests and diagnostics are completed. Providing direct nursing care with professionalism and respect. Participating in care plan implementation and evaluation to meet quality standards. Qualifications Include Current RN license in the specified state. At least two years of geriatric nursing experience. Proficiency in Microsoft Office tools. Ability to travel locally using a personal vehicle. Strong problem recognition and solution skills.
