We pride ourselves on a collaborative culture where clinical excellence and patient-centered care come first.### Position OverviewWe are seeking a compassionate, skilled, and motivated Physical Therapist (PT) to join our rehabilitation team full-time. In this role, you will evaluate, plan, and administer personalized physical therapy programs to help our residents achieve their maximum functional mobility. You will work closely with a multidisciplinary team of doctors, nurses, and fellow therapists to deliver top-tier post-acute care.### Key Responsibilities* Conduct comprehensive initial evaluations of residents to determine their physical condition, limitations, and rehabilitation potential.* Develop, implement, and modify individualized plans of care based on resident needs and physician orders.* Provide high-quality evidence-based physical therapy treatments (including strength training, balance exercises, and mobility training).* Accurately document resident progress, treatments, and discharge plans in compliance with federal, state, and facility guidelines.* Educate residents and their families on post-discharge exercises, safety protocols, and assistive devices.* Collaborate effectively with the interdisciplinary care team to optimize patient outcomes.### Qualifications & Requirements* Education: Master’s or Doctorate degree in Physical Therapy from an accredited program.* Licensure: Active, unrestricted Physical Therapist license in the State of California.* Certification: Current CPR/BLS certification.* Experience: Experience in a skilled nursing facility (SNF), post-acute care, or outpatient rehab setting is preferred, but passionate new graduates are welcome to apply!* Strong communication, interpersonal, and time-management skills.### What We Offer (Benefits & Perks)We value our therapists and believe in taking care of the people who take care of our residents.
