Clinical Pharmacist

Position type: Fulltime/ part-time contract

Est. pay range: $55.00/ hour - $57.43/ hour on W2 USD

Schedule: 40 hours per week, hours will be based on business needs / flexible scheduling - Hiring Manager will consider candidates who only wants part-time work

Position Overview

The pharmacist will be responsible for medication therapy management (MTM) services, including outbound calls for comprehensive medication reviews (CMR) and delivering in-depth consultations on statin medication use per AHA guidelines. The pharmacist will transcribe patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application. The data to be transcribed will be either hard-copy or available from a Health application. The data entered by the pharmacist will be used to create a standardized document that will be mailed to individual patients. The pharmacist is a member of the MTM product team.

Qualifications

Education/Licensing Requirements:

Valid Pharmacist License in Florida

Experience Requirements:

At least 4 years of experience as clinical MTM pharmacist. (Management experience preferred) Familiar with driving Part D Star measures. Specifically, SUPD, SPC, and CMR. Understanding of pharmaceutical and medical vocabulary Clinical experience in medication review.

Excellent verbal and written communication skills

Experience using salesforce preferred

Must be tech savvy

Essential Job Responsibilities

Conducting outbound phone calls. (100 calls per day)

Completing comprehensive medication reviews

Conducting medication therapy management services

Accuracy and speed in entry of patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application
