Position Registered Nurse, Correctional Facility (RCNF) located at California State Prison, Sacramento (SAC) in Sacramento County, CA. Responsibilities Under the supervision of the Supervising Registered Nurse IVIII, the Resource Registered Nurse organizes and provides necessary nursing care for patients participating in Complete Care Model (CCM) initiatives such as Medication Assisted Treatment (MAT) and Hepatitis C (HCV) treatment. The Resource RN serves as the Care Manager for patients with chronic care management needs and works collaboratively with the primary care team, addiction medicine team, and HCV treatment team to ensure timely delivery of appropriate health care. Responsibilities include assessing side effects, signs/symptoms of recent drug or alcohol use or withdrawal, mental health, and medical conditions interfering with treatment; facilitating recovery, physical health, and wellness; medication management groups; promoting therapeutic relationships; mentoring patient peer development; attending primary care meetings and population management sessions; collaborating with the Primary Care Nurse on care visits and planning; administering injections; providing high-level education; serving as a tele-presenter for the Telemedicine Addiction Provider; and addressing ongoing health care needs for patients while promoting efficient use of health care resources. Minimum Qualifications Current license as a Registered Nurse in California. Pass the examination for the REGISTERED NURSE, CORRECTIONAL FACILITY classification.
