Job Description: We are looking for*Physician Assistants and/or Nurse Practitioners to provide primary care services as part of our virtual primary care team. The Physician Assistant/Nurse Practitioner will work with a multi-disciplinary team to care for and provide medical services to patients, including examining/conducting tests; analyze records, reports, and test results; to prevent and/or diagnose illness, disease or injury; prescribe/administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, and/or injury; explain procedures and test results on prescribed treatment plan; manage labs, referrals. The Physician Assistant/Nurse Practitioner will collaborate with other physician assistants, nurse practitioners, physicians, and other support staff as part of the clinical team.**Perform other job-related duties as required by departmental needs/demands. * Experience and Skills: Completion of an approved/accredited educational program for Physician Assistants* Valid NY State license, CDS, DEA, and*BLS Ability to effectively communicate, follow established procedures and protocols Understanding of and passion for Community Healthcare is preferred Benefits: At Urban Health Plan, we value our team and foster a positive, fun work environment!
