divh2Home Health LPN/h2pWere looking for a Home Health LPN to provide practical nursing services under the supervision of a Registered Nurse./ph3Responsibilities/h3pAccepts assignments of clients from the R.N. Case Manager/ppProvides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans/ppPerforms treatment(s) ordered by the physician and delegated by the R.N. Case Manager/ppObserves signs and symptoms and reports them to the R.N. Case Manager/ppNotes changes in physical or emotional status of clients/ppPromotes client and caregiver independence in self-care/ppUtilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of data/ppFollows Agency policies regarding universal precautions, and safety guidelines/ppFulfills other assignments as requested by the R.N.
