Home Health Care Coordinator

NYU Langone Health

Kindred at Home seeks a Patient Services Coordinator to schedule visits and coordinate care for patients in home health settings. You will communicate with field staff, patients, physicians, and the office to maintain continuity of care and smooth operations across the branch. The role requires strong communication, organization, and at least 1 year of home health experience.
