Licensed Practical Nurse (LPN)

Location: Brooklyn, NY

Facility Setting: Outpatient Clinic

Job Type: Full-Time

Responsibilities:

• Prepare patients for examinations and medical visits.
• Assist physicians and advanced practice providers during procedures.
• Administer medications, vaccines, and treatments as ordered.
• Monitor patient conditions and report changes appropriately.
• Educate patients on health conditions, treatment plans, and wellness practices.
• Coordinate referrals and follow-up care with specialty providers.
• Maintain accurate documentation in the Electronic Medical Record (EMR).
• Assist with routine screenings and clinical procedures.
• Ensure compliance with infection control and HIPAA regulations.
• Maintain clinical supplies and medication inventory.
• Support care coordination and pre-visit planning activities.
• Float between clinic locations as needed.

Qualifications:

• Graduate of an accredited Practical Nursing program.
• Current New York State Licensed Practical Nurse (LPN) license.
• Minimum one year of clinical nursing experience preferred.
• Current BCLS Certification required.
• Completion of Infection Control and Child Abuse training required.
• Bilingual (Spanish or Creole) preferred.
• Strong communication and patient care skills.
