1. Associate's Degree from an SLPA program or Bachelor's Degree in Communicative Disorders
2. Resume
3. Successful speech pathology experience preferred
4. (3) Letters of Recommendation
5. TB Test Results or Adult Tuberculosis (TB) Risk Assessment Questionnaire (within 60 days of hire) Valid TB
