Hospital Outreach Manager (Account Manager) Houston, TX

Overview Posting Date : 04/15/2026 Location: Houston, Texas (hybrid role; candidate must live in Houston, TX). Onsite travel up to 40% within Houston. Account manager, account executive and/or project management experience preferred. This role is for a Hospital Outreach Manager at DaVita. It focuses on engaging renal discharge planners and case managers to build and strengthen partnerships and to ensure a smooth transition for newly diagnosed and hospitalized dialysis patients to the outpatient setting. General Purpose Goal is to ensure DaVita is the provider of choice to hospitals and to support patient placement with extenuating circumstances as needed. Responsibilities Analyze data insights to highlight importance and drive growth. Demonstrate executive presence when delivering insights to upper leadership with clear, impactful communication. Leverage a strategic mindset to perform root cause analysis and identify opportunities based on patient feedback. Consistently outreach to discharge planners to provide education and build awareness of offerings. Prioritize key insights and opportunities to align with business goals and stakeholder needs. Identify opportunities to grow as provider of choice through relationship building and addressing feedback. Partner with internal field teammates to optimize the admission process and implement best demonstrated practices. Assist with communication between hospital, admission and clinic teammates to coordinate patient placements with extenuating circumstances. Support a smooth and seamless discharge experience for patients and hospital staff to DaVita centers. Develop comprehensive knowledge of local areas. Required Qualifications Bachelor’s Degree preferred. Ability to travel to various locations to meet with internal and external partners as needed. Travel: 30-40%. Includes valid driver’s license in state of residence and insurable driving record. Intermediate computer skills with Microsoft Word, Excel, PowerPoint, billing systems.
