$50,000 Sign On Bonus Available!
- Full-time, permanent, Monday-Friday schedule
- Provide direct support and case management services to assigned clients
- The Social Worker will monitor and improve the patient experience by focusing on quality, timeliness, and continuity of care
- Must be a Licensed Social Worker (LICSW candidates are highly favored)
- Background in hospital setting is preferred, but not required
- Located in a scenic, rural community offering a peaceful and close-knit environment
- Serves a diverse patient population with meaningful community impact
- Surrounded by beautiful natural landscapes and wide-open spaces
