• Estimated Weekly Pay: $2,030 - $2,190
• Location: Meyersdale, PA (serving Somerset County and nearby communities)
• Contract Duration: 13 weeks (April 26, 2026 - July 25, 2026)
• Shift: 8-hour shifts, 8:00am - 4:30pm
• Hours per Week: 40 (32 hours guaranteed)
• Employment Type: Travel Physical Therapist / On-Site Home Health
• Mileage: Reimbursed between patient visits
• Dress Code: Royal blue/black scrubs or khakis and polo shirt

Travel PT Job Requirements

• Active Physical Therapist (PT) license valid in Pennsylvania
• Physical Therapist degree (Associate's or higher)
• Minimum 1 year of clinical experience as a Physical Therapist; home health or rehab experience preferred
• Comfortable traveling within Somerset County and surrounding areas with reliable transportation
• Strong communication, documentation, and organizational skills-experience with OASIS documentation a plus
• Ability to meet productivity expectations and manage caseload in a home health setting

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized care plans as a Home Health Physical Therapist
• Perform start of care assessments, OASIS visits, and return/supervisor visits
• Deliver high-quality rehabilitative therapy services in patients' homes across Somerset County
• Meet productivity expectations (30 points per week) and manage documentation in a timely manner
• Collaborate with interdisciplinary teams (nursing, case management, social work) to ensure optimal patient outcomes
• Maintain accurate, timely clinical documentation and compliance with home health policies
• Participate in weekend/on-call rotation as discussed (approximately every 8 weekends)
