• Provide direct patient care in a home health setting
• Collaborate with healthcare team to develop and implement personalized care plans
• Monitor and document patient progress and response to treatment
• Deliver exceptional patient and family education
• Ensure all clinical protocols and standards are maintained

Requirements of the Registered Nurse - Home Health

• Reg Nurse (Single State) or RN - Multi-state Compact
• Basic Life Support or BLS - Instructor
• PALS certification required
• Diploma from an accredited nursing program; BSN preferred
• Less than 1 year of experience in direct patient care
• Strong interpersonal and communication skills
